Please present a valid insurance card and photo ID at each visit. 

Co-Payments
If your insurance carrier requires a co-payment on the services you receive, its due at the time of service.

Medicare Coverage
Medwin Primary Care is a participating provider with Medicare. A second insurance is filed after Medicare when the information is given in advance.

HMO Coverage
Dr. Chadalawada must be the primary care physician with your insurance company. You will not receive a bill unless your carrier denies payment. You will be responsible for your co-pay at the time of visit.

PPO Coverage
If Dr. Chadalawada participates in your plan, we will submit your claim to the PPO insurance. If there is no response within 90 days, the balance becomes immediately due and payable by the patient. You are required to pay your co-pay at the time of service.

Secondary Insurance
We will file with your secondary insurance unless the balance is a co-pay. This is done after payment is received from your primary insurance. If there is no response within 90 days, the balance becomes immediately due and payable by the patient.

Patient Statements
We send statements to patients monthly.  We accept the following forms of payment,

• Personal Checks
• Visa
• MasterCard
• Cash

We accept payment upon request and prefer a phone call in financial hardship, to prevent collections.
